1. Introduction
These Terms and Conditions (“Terms”) govern your use of the NDIS Provider Group website and the services provided by NDIS Provider Group (“we,” “us,” “our”), a registered NDIS provider based in Sydney, New South Wales, operating since 2012. By accessing our website, enquiring about our services, or engaging us as your NDIS provider, you agree to be bound by these Terms.

2. About Our Services
NDIS Provider Group is a registered NDIS provider offering a range of disability support services across Sydney and other Australian locations, including but not limited to Support Coordination, Personal Care, Daily Living Support, Community Participation, Accommodation Supports, Nursing and Complex Care, Allied Health coordination, and related services as described on our website.
All services are delivered in accordance with the NDIS Act 2013, the NDIS Practice Standards, and the requirements of the NDIS Quality and Safeguards Commission.
3. Eligibility and Service Agreements
3.1 Access to our services requires an active NDIS plan with relevant funding allocated to the supports you wish to receive.
3.2 Before commencing services, we will work with you to establish a Service Agreement, which outlines the specific supports to be provided, pricing in line with the NDIS Pricing Arrangements and Price Limits, and the responsibilities of both parties.
3.3 Services will only be delivered in accordance with your NDIS plan and applicable funding categories. We reserve the right to decline services that fall outside your plan’s funding or that we are not appropriately qualified or registered to provide.
3.4 You are responsible for ensuring that your NDIS plan has sufficient funding available for the services you request.
4. Fees and Payment
4.1 Our fees are charged in accordance with the current NDIS Pricing Arrangements and Price Limits, as published by the National Disability Insurance Agency (NDIA).
4.2 Depending on how your NDIS plan is managed (NDIA-managed, plan-managed, or self-managed), payment for services will be processed through the appropriate channel, either directly through the NDIA portal, via your plan manager, or by invoice to you.
4.3 We will provide clear invoicing for all services delivered. Please review invoices promptly and contact us if you have any queries or concerns.
4.4 Cancellation fees may apply in accordance with the NDIS Pricing Arrangements if a scheduled service is cancelled with insufficient notice. Current cancellation policy details will be outlined in your Service Agreement.
5. Cancellations and Changes to Support
5.1 We understand that circumstances change. If you need to cancel or reschedule a service, please provide as much notice as possible, in line with the notice period specified in your Service Agreement.
5.2 Late cancellations or non-attendance may incur a cancellation fee, consistent with NDIS pricing guidelines, to cover costs associated with support worker scheduling.
5.3 We may need to reschedule or adjust services in exceptional circumstances, such as staff illness, safety concerns, or unforeseen operational issues. We will make every reasonable effort to provide notice and arrange suitable alternative support.
6. Participant Rights and Responsibilities
6.1 As a participant, you have the right to:
- Be treated with dignity, respect, and courtesy at all times
- Receive services free from discrimination, exploitation, neglect, and abuse
- Be involved in decisions about your own supports and goals
- Provide feedback or make a complaint without fear of retribution
- Access your personal information in accordance with privacy laws
6.2 As a participant, you agree to:
- Provide accurate and up-to-date information relevant to your support needs
- Treat our staff with respect and courtesy
- Provide a safe working environment for support workers attending your home
- Notify us promptly of any changes to your NDIS plan, contact details, or support needs
7. Our Responsibilities
7.1 We are committed to delivering services in accordance with the NDIS Practice Standards and Code of Conduct, including:
- Providing services that are safe, competent, and respectful
- Ensuring all support workers are appropriately screened, qualified, and trained
- Maintaining appropriate insurance and compliance with all relevant regulations
- Protecting your privacy and handling your personal information responsibly
7.2 We will communicate openly and honestly with you regarding your services, any concerns, and any changes that may affect your support.
8. Complaints and Feedback
8.1 We take all feedback and complaints seriously and are committed to resolving concerns promptly and fairly.

11.1 Either party may terminate a Service Agreement by providing reasonable written notice, as specified in the relevant Service Agreement.
11.2 We reserve the right to suspend or terminate services immediately in circumstances involving safety risks to our staff, breach of these Terms, or non-payment of fees, subject to our obligations under the NDIS Practice Standards.
